Background: Chronic pain is a complex condition with significant psychological and emotional dimensions, often associated with impaired emotion regulation. While psychological interventions such as Cognitive Behavioral Therapy (CBT) can alleviate some symptoms, many patients continue to experience persistent emotional distress. Emotion regulation has been identified as a key modifiable mechanism in adapting to chronic pain; however, its delivery beyond clinical settings remains limited. This study proposes a conceptual framework for designing a real-time artificial intelligence (AI) assistant to support emotion regulation in patients with chronic pain.
Method: Using an analytical synthesis approach, key concepts from three domains—pain psychology, emotion regulation, and AI technology—were collected and integrated. Through identifying thematic patterns and conceptual bridges among these areas, a theoretical model was developed with a focus on the practical application of AI to facilitate emotional self-regulation.
Findings: The proposed AI assistant utilizes conversational agents and wearable sensors to detect emotional states related to pain and deliver timely emotion regulation interventions such as cognitive reappraisal, fear reduction, and attentional redirection. The system functions not only as a digital intervention tool but also as a communicative interface that enhances trust, autonomy, and flexibility.
Conclusion: The presented framework demonstrates the potential of AI-based assistants to play a key role in chronic pain management by targeting emotional mechanisms. Developing a prototype and evaluating its real-world effectiveness are the next steps in advancing this innovative approach.
