fa مقایسه تاثیر بیهوشی عمومی و بیحسی اسپاینال بر قندخون بعد از عمل سزارین Comparison the effect of general and spinal anesthesia on blood sugar after cesarean section surgery بیهوشی رژیونال Regional anesthesia كاربردي Applicable مقدمه: یکی از پیامدهای جراحی های شایع و بزرگی مانند عمل سزارین هیپرگلیسمی می باشد که با عوارض زیادی از جمله به تاخیر افتادن بهبود زخم ها و همچنین ایجاد عوارض در سیستم های حیاتی بدن و افزایش مورتالیتی و موربیدیتی می باشد. بنابراین پژوهش حاضر با هدف مقایسه تاثیر بیهوشی عمومی و بیحسی اسپاینال بر قندخون بعد از عمل سزارین انجام شد. مواد و روش: این پژوهش یک مطالعه تحلیلی – مقایسه ای است که بر روی40 نفر از بیماران که جهت انجام عمل جراحی سزارین به بیمارستان علامه بهلول گناباد و بیمارستان شهدا قاین مراجعه کردند انجام شد. درگروه اسپاینال، بیحسی نخاعی درسطح مهره های کمری سوم و چهارم یا چهارم و پنجم به وسیله بوپیواکایین با استفاده از سوزن شماره 24 انجام شد و درگروه بیهوشی عمومی با روش یکسان و داروهای مشابه بیهوش شدند و تحت عمل جراحی سزارین قرار گرفتند. به دلیل تاثیری که اضطراب بر قند خون دارد، پرسشنامه مربوط به اضطراب برای سنجش اضطراب قبل از عمل توسط بیمار تکمیل شد. قندخون بیماران در هر دو گروه به وسیله گلوکومتر سی دقیقه قبل از جراحی )در اتاق انتظار(، و در حین عمل و همچنین سی دقیقه بعد از اتمام جراحی )در اتاق بهبودی(  اندازه گیری و ثبت و از نظر آماری مورد تحلیل و مقایسه قرار گرفت. یافتهها: نتایج نشان داد که دو گروه از نظر متغیرهای دموگرافیک همگن بودند(05/0 <P). نتایج آزمون آنالیز واریانس با تکرار مشاهدات نشان داد اختلاف معنیداری بین دو گروه مطالعه از نظر میانگین قند خون قبل از عمل جراحی سزارین، حین و بعد از عمل جراحی وجود نداشت (05/0 <P). اما در طول زمان قند خون در هر دو گروه افزایش یافت و این افزایش در بعد از عمل نسبت به قبل و حین عمل معنی دار بود (001/0 >P). نتیجهگیری: قند خون بیماران در طی سزارین افزایش معنی داری در هر دو گروه داشته اما بین دو گروه تفاوت معنی داری وجود ندارد که این نشان می دهد نوع بیهوشی تاثیر چندانی بر قند خون بیماران تحت عمل سزارین ندارد.   Introduction: One of the consequences of common and major surgeries such as cesarean section is hyperglycemia, which has many complications, including delayed wound healing, complications in vital body systems, and increased mortality and morbidity. Therefore, the present study was conducted to compare the effects of general anesthesia and spinal anesthesia on blood sugar after cesarean section. Materials and Methods: This study is an analytical-comparative study conducted on 40 patients who referred to Allameh Bahloul Gonabad Hospital and Shohada Qain Hospital for cesarean section. In the spinal group, spinal anesthesia was performed at the level of the third and fourth or fourth and fifth lumbar vertebrae using bupivacaine using a 24-gauge needle, and in the general anesthesia group, they were anesthetized with the same method and similar drugs and underwent cesarean section. Due to the effect of anxiety on blood sugar, an anxiety questionnaire was completed by the patient to measure preoperative anxiety. Blood sugar levels of patients in both groups were measured and recorded using a glucometer thirty minutes before surgery (in the waiting room), during surgery, and thirty minutes after surgery (in the recovery room), and were statistically analyzed and compared. Results: The results showed that the two groups were homogeneous in terms of demographic variables (P < 0.05). The results of the analysis of variance with repeated observations showed that there was no significant difference between the two study groups in terms of mean blood sugar levels before, during, and after cesarean section (P < 0.05). However, blood sugar levels increased in both groups over time, and this increase was more significant after surgery than before and during surgery (P < 0.001).
