fa مقایسه تاثیر بلوک عصب اینترکوستوبراکیال با بلوک عصب آگزیلاری بر میزان درد تورنیکه در بیماران تحت عمل جراحی اندام فوقانی با بلوک اینفرا کلاویکولار تحت هدایت سونوگرافی، یک کارآزمایی بالینی تصادفی شده یک سو کور Comparison between intercostobrachial nerve block and axiliary nerve block on tourniquet pain in patients under upper limb surgeries who received infraclavicular block under guide of ultrasound: a randomized single blind clinical trial study بیهوشی رژیونال Regional anesthesia كاربردي Applicable چکیده زمینه و هدف: هدف از پژوهش حاضر مقایسه تاثیر بلوک عصب اینترکوستوبراکیال با بلوک عصب آگزیلاری بر میزان درد تورنیکه در بیماران تحت عمل جراحی اندام فوقانی با بلوک اینفرا کلاویکولار بود. روش کار: این مطالعه، یک مطالعه کارآزمایی بالینی تصادفی شده یک سو کور است. پس از کسب رضایت،.30 بیمار به ‌صورت تصادفی به 2 گروه تقسیم شدند. به گروه اول (15 نفر) تحت بلوک اینترکوستوبراکیال 5 سی سی از روپیواکائین 5/0٪  و در گروه دوم (15 نفر) تحت بلوک عصب آگزیلاری 5 سی سی از روپیواکائین 5/0٪ بصورت زیر جلدی تزریق شد. میزان رضایت بیماران از طریق پرسشنامه اندازه گیری و داده‌ها با استفاده از نرم spss نسخه 26 تجزیه و تحلیل شد. یافته ها: 30 بیمار (15 مرد و  15زن) بودند. میانگین سنی دربلوک Axillary 13.62  ± 42.33 و در بلوک Intercostobrachial 17.71± 48.07 بود. هیچ یک از بیماران در هر دو گروه درد در ناحیه بستن تورنیکه را گزارش نکردند و نیازی به دریافت مخدر نداشتند. تعیین میزان رضایتمندی بیماران دردو گروه اختلاف معنی داری نداشت (P-Value=.553). توزیع میانگین فشار سیستولی (P-Value=0.789) و دیاستولی  (P-Value=0.942) .در قبل و بعد از بستن تورنیکه بعد از 4 بار اندازه گیری اختلاف معنی داری را نشان نداد. نتیجه گیری: یافته ها نشان می دهد استفاده از بلوک اینترکوستوبراکیال و عصب آگزیلاری موجب از بین رفتن درد در ناحیه بستن تورنیکه می شود و نیاز به داروهای مخدر سیستمیک نخواهد بود. بیشتر بیماران از عمل جراحی رضایت عالی داشتند.   Abstract Background and Aim: The aim of the present study was to compare the effect of intercostobrachial nerve block versus axillary nerve block on the level of tourniquet pain in patients undergoing upper limb surgery with infraclavicular block. Material and Method: This study was a randomized, single-blinded clinical trial. After obtaining consent, 30 patients were randomly divided into 2 groups. The first group (15 patients) received 5 cc of 0.5% ropivacaine for intercostobrachial nerve block, and the second group (15 patients) received 5 cc of 0.5% ropivacaine for axillary nerve block subcutaneously. Patients' satisfaction was measured using a questionnaire, and the data were analyzed using SPSS version 26 software. Results: 30 patients (15 men and 15 women) were included. The mean age was 42.33 ± 13.62 years in the Axillary block group and 48.07 ± 17.71 years in the Intercostobrachial block group. None of the patients in either group reported pain in the tourniquet area and did not require opioid administration. The determination of patient satisfaction level did not show a significant difference between the two groups (P-Value=0.553). The distribution of mean systolic (P-Value=0.789) and diastolic (P-Value=0.942) blood pressure before and after tourniquet application after 4 measurements did not show a significant difference. Conclusion: The findings indicate that the use of intercostobrachial and axillary nerve blocks results in the elimination of pain in the tourniquet area, and there is no need for systemic opioid medications.
