Aim and Background: Cystic hygroma is a congenital malformation of the lymphatic system in which obstruction between the lymphatic and venous pathway in fetal neck leads to lymph accumulation in the jugular sac of nuchal region.

Case report: A 8- years- old infant with a huge anterior cervical mass is presented. He became candidate for tumor excision under general anesthesia as the mass did not regress after pharmacologic management and intra- tumoral injection. The patient was first examined under direct laryngoscopy, being awake then inhalational induction with Sevoflurane was used. At last and after extubation at the end of surgery, he was transferred to intensive care unit with stable condition and awake.
